Clinical details, diagnoses, treatment notes, and similar data should only be submitted through approved systems. Public marketing forms on this site are designed for non-PHI lead capture (name, phone, high-level interest).
Third-party forms (e.g. JotForm)
If BHS uses JotForm or similar tools for applications or clinical documentation, those accounts and submissions are operated by BHS (or its chosen vendor), not by this website’s host. Confirm any HIPAA-eligible plan and Business Associate Agreement (BAA) directly with that vendor and your counsel. This marketing site defaults to phone and email so PHI is not required to contact BHS online.

This static site may use privacy-friendly analytics in the future. Avoid third-party trackers that conflict with healthcare privacy expectations. Prefer first-party or BAA-covered tools.
Access & security
